Why I collect your personal data and what is done with it.
When you supply your personal data to Enchanting Beauty, it is stored and processed for the following reasons :
I need to collect some personal information about you & your health in order to make sure there are no contraindications to your treatment & to be covered by my insurance. You can refuse to provide this information, however, sadly I would have to refuse your treatment as this information is a legal requirement for my insurance. I must keep a record of your name, date of birth, medical history, allergy patch tests & treatment notes for insurance purposes.
If you have given your consent I may occasionally send you e-mails with special offers, treatments news or to let you know my annual leave dates. These emails will be sent securely via MailChimp & you can easily choose to stop receiving these e-mails by clicking “unsubscribe” at the bottom of each e-mail or by requesting that I delete all information from the mailing list.
If you have given your permission to give me your contact number, this will only be used to contact you about any booked appointments (for reasons such as illness, information about your booked treatments or if I am running late on the day of your appointment).
I have a legal obligation to retain your records for 7 years after your most recent appointment for insurance purposes.
For clients under the age of 18, records must be kept for 7 years after they reach the age of 18 for insurance purposes. Parental or guardian consent must be given for under-18’s.
After this time, your records will be destroyed/deleted completely if no longer needed for your treatment.
Paper records are stored in a locked cabinet in my home & are only accessible by myself (Sarah Smith - Enchanting Beauty).
Digital records are completed on JotForm. These are password protected & only accessible by myself (Sarah Smith - Enchanting Beauty).
Your data will not be sold or shown to any third parties. I (Sarah Smith - Enchanting Beauty) am the only person who has access to your data.
You have the right to request a copy of the personal data I hold on you & you can also ask me (Sarah Smith - Enchanting Beauty) to correct any factual errors. You can also request that I destroy all data I hold on you, but I can only do this if 7 years has passed since your most recent appointment for insurance purposes. Address, phone number & e-mail address details can be destroyed.
I would like you to be absolutely confident that I treat your personal data responsibly. If you feel I have mishandled your data in some way, please contact me to discuss & you have the right to make a formal complaint.
